Job Summary
Assist in the day-to-day operation of residences providing supervised and supported living for persons with behavioral health problems by participating in the Interdisciplinary Team to recommend service plans and implement consumer center plans. Train, mentor, and ensure new direct service staff are competent in job duties and electronic medical records usage. Provide medication oversight, document self-administration, and ensure proper storage of consumer medications. Monitor each consumer's physical and mental status for early signs of decompensation, assist with programmatic assessments, and facilitate transition services for consumers moving into or out of the residential program. Enhance consumer independence and decision-making ability through active treatment and normalization while supporting orientation, training, and community provider appointments.
Required Qualifications
- High School Diploma/GED
- 21 years of age or older
- Valid driver's license
- Reliable transportation
- Meets organization's insurability requirements
- Department of Mental Health (DMH) credentialing requirements of a Rehabilitative Services Assistant (RSA)
- Completion of all background checks
Desired Qualifications
- Minimum 2 years documented experience working with persons with behavioral/mental disabilities
- One or more years of human service experience
