Job Summary
Provide direct care to persons with developmental and physical disabilities in our Supportive Living Services Program. Implement behavior programs, habilitation strategies, and exercise plans while facilitating community integration and safety. Address medical health issues by scheduling appointments, providing transportation, and communicating pertinent health updates to supervisors. Assist individuals with daily hygiene functions and special prescribed diets, including food preparation and eating procedures. Conduct hourly safety checks for overnight awake staff and model positive adult roles in the community. Foster belonging through person-centered activities and assist in completing individual assessments.
Required Qualifications
- High school diploma or general education degree (GED)
- 3 to 6 months related experience and/or training
- human service training certificate
- Valid Connecticut Driver's License
- Department of Developmental Services - Medication Certification (to be obtained within six months of hire)
- Public Service License valid for the state of Connecticut (if applicable)
- CPR and First Aid certification
