Job Summary
Assist individuals with daily living skills including shopping for clothing, grooming, meal preparation, laundry, and budgeting. Monitor personal hygiene and homemaking abilities while encouraging independence in all areas. Provide transportation to recreational events and community resources, ensuring defensive driving and seat belt use. Administer medications as directed, count supplies, and schedule medical appointments with appropriate documentation. Maintain home cleanliness, manage fire safety protocols, and perform yard upkeep. Record thorough logs for all activities, behaviors, and incidents. Communicate concerns to the team and follow program plans for each resident.
Required Qualifications
- High school diploma or GED
- Valid driver's license
- Access to insured vehicle
- Proof of a good driving record
- Must attend all behavior management training
- Must attend annual nursing in-service
- Must attend annual fire safety training
- Responsible to the Manager/Service Coordinator for the routine fire and safety needs of all individuals
- Responsible for completing appropriate nursing in-services
- Responsible for completing incident/accident reports as appropriate and submitting to the appropriate individuals (Manager, Service Coordinator, Nurse)
- Responsible for the overall cleanliness of the home (inside and outside) as well as company vehicles
- Responsible to keep company vehicles clean and to follow the no eating or drinking rules
- Responsible to follow the rules of the road and to drive defensively
- Responsible to ensure each individual wears a seat belt when being transported
- Responsible for completing daily chores
- Responsible for gathering data and participating, as appropriate, in the annual GSP/ISP conference
- Responsible for gathering data as outlined in program plan
- Responsible for following program plans for each individual
- Responsible for documentation/data collection for all behavior
- Responsible for bringing to the Manager/Service Coordinator any concerns regarding significant changes in individual's behavior
- Responsible for completing pre-GSP tools (i.e., recreation assessments skills assessments, etc.)
- Responsible for documentation in the house & of all recreational activities
- Responsible for ensuring all appropriate documentation is completed and submitted to the appropriate people (interagency forms)
- Responsible for completing all medical appointments
- Responsible for obtaining approval from appropriate supervisor before canceling a medical appointment
- Responsible for providing food/nutrition services in compliance with policies
- Responsible for planning menus as appropriate and having them approved by the RN/Dietitian
- Responsible for ensuring that all food is labeled and stored appropriately
- Responsible for ensuring food is prepared according to the individual needs/tastes
- Responsible for ensuring that a sufficient quality of food is available
- Responsible for assisting individuals in the planning, preparation and serving of meals
- Responsible for assisting individuals with clean up after meals
- Responsible for assisting individual with their grocery shopping
- Responsible for administering medication as appropriate
- Responsible for counting identified medications at the beginning and end of the shift
- Responsible for following all medication/treatment as given
- Responsible for signing off for all medication/treatment as given
- Responsible for filling/refilling medications as directed at the appropriate pharmacy
- Responsible for storing medications appropriately
- Responsible for providing transportation to and from desired recreational events
- Responsible for providing transportation to and from work for each individual
- Responsible for checking the house for potential hazards/defects at the start of each shift
- Responsible for notifying the appropriate person of potential hazards/defects
- Responsible for performing fire drill evacuation according to schedule
- Responsible for documenting response of individual to fire drill/evacuation procedures
- Responsible for encouraging appropriate use leisure time
- Responsible for planning and implementing recreational activities remembering individuals limited financial resources
- Responsible for instructing appropriate social skills
- Responsible for instructing appropriate community skills
- Responsible for determining individual's interests and assisting them in participating in/developing these interests
- Responsible for encouraging hobbies
- Responsible for assisting individuals in use of community resources and becoming an active member of their community
- Responsible for assisting individuals in practicing their religious belief
- Responsible for assisting each individual to function at their maximum in money management (budgeting, banking, shopping)
- Responsible for assisting individuals to select an attractive hairstyle and assist them in maintaining it
- Responsible for monitoring (as appropriate) personal Hygiene skills and offering assistance as needed
- Responsible for assisting/monitoring each individual with homemaking skills (laundry, meal prep, chores, etc.)
- Responsible for assisting individuals to develop a positive self-image
- Responsible for treating each individual with respect and dignity
- Responsible for treating each person as an individual who has different needs and wants
- Responsible for assisting individuals to shop for appropriate clothing (discuss with individual appropriate clothing for the occasion, appropriate appearance)
- Responsible for encouraging individuals to have routine contact with family and friends in order to maintain positive relationships (i.e., regular visits to individuals' home, birthday recognition, regular phone calls)
- Responsible for encouraging individuals to be independent in all areas of their life (i.e., homemaking skills, recreation activities, personal hygiene, employment, etc.)
- Responsible for working with all staff in developing a warm and supportive environment for growth and learning
- Responsible for reading the log at the start of each shift
- Responsible for writing in logs on each shift information pertinent to the individuals and/or running of the home
- Responsible for supporting and carrying out team decisions consistently and appropriately
- Responsible for implementing behavior management programs, as appropriate for each individual
- Responsible for providing comfort and reassurance to individual during appointments
- Responsible for scheduling and carrying through all medical appointments when necessary
- Responsible for ensuring that a sufficient quality of food is available
- Responsible for lifting 100 pounds with two people
- Responsible for standing 80% of the shift
- Responsible for bending 50% of the shift
