Job Summary
Provide psycho-educational and supportive therapeutic interventions to adults with MH/SA disabilities in a residential group-home setting. Deliver recovery-oriented strategies to support wellness, self-management, independent living skills (housing, employment, and community inclusion), daily living activities, and coping skills. Collaborate with the interdisciplinary team to monitor client progress, document services, administer and report vital information as required, assist with transportation, shopping, meal preparation, housekeeping, intake/tenant processes, medication observations (MARs/prescriptions coordination), crisis interventions, and participate in team planning. Maintain confidentiality per HIPAA and follow the PCP/service plan and clinical orders. Work both independently and as part of a team to support clients 18+ in a supervised living facility, with a focus on functional problem-solving across vocational, educational, personal care, domestic, psychosocial, and communication domains.
Required Qualifications
- High School Diploma or GED
- Para Professional, Associate Professional or Qualified Professional
- Valid driver's license
- At least twenty-one years old
