Job Summary
Provide direct medical care as ordered by a physician, educate individuals and staff on medical conditions and medications, and deliver individualized training for health and wellness groups. Assist clients in accessing community health resources, review medical documentation for care coordination, and coordinate follow-ups on assigned appointments. Support the team in assessing symptoms, guide individuals in understanding treatment options, and build relationships with community medical providers and VNA services to enhance care coordination. This role requires an RN with experience working with adults with intellectual or developmental disabilities, a valid Massachusetts driver's license, and a personally owned vehicle. Schedule is Monday-Friday, 8am-4pm or 9am-5pm, with locations in Braintree, Stoughton, and North Attleboro.
Required Qualifications
- Associates degree in Nursing
- Experience working with adults with intellectual or developmental disabilities
- Valid Massachusetts driver's license
- Ability to work in the areas of Braintree, Stoughton and North Attleboro
