Job Summary
Deliver person-centred and culturally responsive case management support to individuals accessing residential mental health services. Conduct intake, assessments, and registrations while assisting clients with daily living activities and recovery goals. Develop holistic, strengths-based individualised case plans and facilitate referrals to specialist services. Maintain accurate client records, reports, and case management documentation. Participate in team meetings, training activities, and practical group-based program sessions. Build positive relationships with clients, colleagues, and external stakeholders to promote recovery and community integration. Operate across a 24/7 roster including days, nights, weekends, and public holidays. This max term role concludes on 30th June 2027.
Required Qualifications
- Strong understanding of mental health, recovery-orientated practices
- Degree in social work or related discipline
- A positive person-centred approach with strong communication and interpersonal skills
- Strong organisational and administrative skills
- Resilience and ability to work with complex environments
- National police history check
