Job Summary
Conduct person-centered clinical assessments to determine education levels, monitoring needs, and support requirements for residents. Coordinate treatment planning with the CPRP and Healthcare Home team to monitor progress in community living, vocational programs, and organized treatment. Lead recovery efforts for individuals with mental illness, chemical dependency, or co-occurring disorders while providing ongoing training and coaching to residential specialists and peer support staff. Maintain clinical and administrative documentation, including electronic health records and billing reports, and participate in quality assurance, accreditation, and risk management activities. Support families in treatment planning and service linkage while promoting consumer independence and self-care.
Required Qualifications
- Masters Degree in Social Work, Nursing, Counseling or related field
- A qualified mental health professional with two years' experience in the mental health field, at least one of those years working with the seriously and persistently mentally ill
- Ability to manage complex personnel and community partnership issues with professionalism, diplomacy and tact
- Strong interpersonal skills, excellent verbal and written communication skills
- Strong consumer advocacy skills and empowerment oriented
- Ability to effectively prioritize and manage simultaneous task
Desired Qualifications
- Clinical license in the State of Missouri preferred
