Job Summary
Develop, facilitate, and maintain Individualized Support Plans while monitoring service implementation and evaluating progress toward meaningful outcomes. Ensure compliance with Indiana Bureau of Disabilities Services regulations, CMS HCBS Final Rule requirements, and agency policies through routine record reviews, documentation audits, and survey preparation. Coordinate interdisciplinary services by leading team meetings, collaborating with families and providers, and advocating for community integration. Conduct quality assurance reviews, analyze incident trends, and recommend process improvements to enhance service delivery and prevent regulatory deficiencies. Provide technical support and training on regulatory requirements to Direct Support Professionals and management. Maintain timely documentation, uphold agency values, and perform other duties as assigned.
Required Qualifications
- Bachelor's degree in Human Services, Psychology, Social Work, Special Education or related field
- Meets Indiana Qualified Disabilities Professional (QDP) qualifications
- Valid driver's license with acceptable driving record
- Ability to pass required criminal background checks
- Maintain CPR/First Aid and all required agency certifications
- Ability to work effectively in both office and residential environments
- Ability to lift or move items up to 75 Ibs
- Ability to walk or stand for extended periods of time
- Ability to respond to emergency situations involving potential physical harm to self or others
Desired Qualifications
- Minimum one year experience supporting individuals with intellectual and/or developmental disabilities
