Job Summary
Facilitate a team approach to quality care for individuals with developmental disabilities, brain injuries, and neurological impairments by acting as a key liaison, advocate, and resource for routine and special healthcare needs. Provide nursing care management oversight while assessing, improving, and maintaining the quality of life and healthcare for persons served in accordance with Nurse Practices Act, DDD, and CARF guidelines. Collaborate with family members, clinical staff, physicians, and regulatory agencies to develop comprehensive care plans and remove health-related barriers. Operate within personal license constraints and established Bancroft policies to ensure continued provision of quality health care.
Required Qualifications
- Registered Nurse
- current and valid N.J. license
- Degree in Nursing
- certification in Case Management/ Developmental Disability and or Brain Injury Specialist
- Certified Rehabilitation Registered Nurse
- CRRN
- two year of general nursing experience
Desired Qualifications
- three years of prior experience
