Job Summary
Assist residents with medical management including physicals, referrals, prescriptions, appointments, and treatments, while providing emergency first aid during crises. Coordinate care with residents, families, physicians, and providers regarding status changes and medication, overseeing direct care staff under the direction of the Residential Program Nurse. Implement, document, and supervise medication management per agency regulations. Monitor care continuity, evaluate quality, and participate in staff meetings, trainings, and resident/family meetings. Requires Massachusetts LPN licensure, one year community healthcare experience, valid driver's license, and an operational vehicle. Schedule is Monday through Friday, 7 a.m. to 5 p.m. with some flexibility.
Required Qualifications
- Massachusetts as a Licensed Practical Nurse
- 1 year experience in a community healthcare setting
- Strong oral and written communication skills with individuals, families, funding sources, consultants, the community at large, and the program and administrative staff
- Excellent written and verbal communication
- Must hold a valid driver's license
- Have access to an operational and insured vehicle
- Be willing to use [the] vehicle to transport clients
