Job Summary
Assist residents with medical management including physicals, referrals, prescriptions, appointments, and treatments while providing emergency first aid during crises. Coordinate care with residents, families, physicians, and the site team regarding medical changes, and oversee direct care staff under the Residential Program Nurse's direction. Implement and document medication management in accordance with regulations, monitor care continuity, and lead quality improvement initiatives. Participate in staff meetings, trainings, service planning, and resident/family meetings. Requires a Massachusetts LPN license with one year of community healthcare experience, strong communication skills, and a valid driver's license.
Required Qualifications
- Massachusetts as a Licensed Practical Nurse
- 1 year experience in a community healthcare setting
- Strong oral and written communication skills with individuals, families, funding sources, consultants, the community at large, and the program and administrative staff
- Excellent written and verbal communication
- Must hold a valid driver's license
- Have access to an operational and insured vehicle
- Be willing to use [the] vehicle to transport clients
