Residential Healthcare Group: Home Health Rn - Evening Shift

Provide evidence-based nursing care to a diverse patient population in an acute care setting. Perform thorough patient assessments, develop and implement individualized care plans, administer medications and treatments, monitor responses and vital signs, document care in the electronic health record, and coordinate treatment with physicians and the interdisciplinary team. Triage care needs, respond to emergencies, and follow infection prevention practices and hospital policies to maintain patient and staff safety. Collaborate with case management, social work, and ancillary departments to support safe transitions of care, participate in unit-based quality and performance improvement initiatives, mentor and support colleagues, and pursue ongoing professional development in a hospital-based, on-site environment.
