Job Summary

Provide daily support, advocacy, and physical assistance to individuals living in a 24-hour residential house while ensuring their health and safety. Assist with community-based experiences, transportation, and medication dispensing, then document all interactions, contact hours, and incidents in Therap ISP data. Respond to emergencies, advocate for community inclusion, and collaborate with the interdisciplinary team to support each person's person-centered plan. Maintain HIPAA compliance, attend staff meetings, and communicate schedule changes to the Residential Supervisor. Work flexible shifts including evenings and weekends, and be prepared to work an overnight shift.

Required Qualifications

  • High School Diploma or equivalent
  • Valid Chauffeur's license
  • driving record that is acceptable to Trivium Life Services auto insurance company
  • Ability to push, pull, lift and/or transfer up to 50 pounds
  • Ability to accurately dispense medications as instructed by Medication Manager
  • Must secure and maintain a private phone
  • Ability to work an overnight shift
  • Ability to travel between locations
  • Must be able to travel between locations
  • Must be able to attend seminars which may require an overnight stay
  • Ability to work weekend and/or evening work
  • Must be able to talk and hear
  • Must be able to sit and use their hands and fingers
  • Must be able to handle or feel and to manipulate keys on a keyboard
  • Must be able to stand, walk, reach with arms and hands
  • Must be able to climb or balance
  • Must be able to stoop, kneel, crouch or crawl
  • Must have close and distant vision
  • Must be able to handle or feel and to manipulate keys on a keyboard
  • Must be able to travel between locations
  • Must be able to attend seminars which may require an overnight stay
  • Must have authorized release before sensitive information is given pertaining to TRIVIUM LIFE SERVICES, persons receiving services, and staff
  • Will be aware of and comply with all HIPAA regulations
  • Will abide by all Trivium Life Services Corporate Compliance policies and Code of Ethics
  • Must secure Certification in CPR and First Aid
