Job Summary
Conduct hands-on daily support for individuals with higher medical and personal care needs, including bathing, grooming, toileting assistance, meal preparation, and mobility support using gait belts or mechanical lifts. Monitor and document health changes, administer medications, and manage specialized tasks such as tracheostomy care, feeding tubes, oxygen therapy, and catheter maintenance as trained. Respond calmly to behavioral concerns and de-escalation situations while maintaining accurate progress notes and ensuring the safety and dignity of residents. This role includes a Skilled Care pay differential of $2.00-$4.00 per hour for specialized site assignments and requires strong attention to health monitoring and problem-solving in a team-based residential environment.
