Job Summary
Assist individuals with daily routines, meal preparation, and personal care while encouraging independence in budgeting and daily planning. Support community engagement by accompanying individuals to appointments, outings, and social activities to build relationships and skills. Monitor health and wellbeing, administer medication per procedures, and report changes in mood or behavior. Complete progress notes, communicate updates to supervisors, and follow individual service plans within small residential homes. Shifts include day, evening, weekend, and overnight options from 8:00 AM to 8:00 AM.
