Job Summary
Conduct routine, preventive, and emergency health care while assisting with hands-on direct care tasks including showering, toileting, shaving, and meal-time assistance. Ensure residential environments remain safe and secure, then provide education, training, and counseling to achieve Individual Service Plan goals in functional academics, domestic skills, hygiene, and social mobility. Develop annual ISPs and semi-annual assessments, perform case management duties, and maintain confidential files. Provide transportation as necessary and communicate with family, guardians, and funding sources to facilitate programming consistency.
Required Qualifications
- Experience working with Individuals with ID/DD
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Valid Driver's License
- Excellent verbal and written communication skills

- Valid MAP certification
