Job Summary
Provides direct care to individuals in a residential setting through supervision, role modeling and teaching. Ed ucation training, counseling, and physical assistance to achieve ISP goals in areas including functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication, community mobility, self-esteem, and money management. Develops and presents annual ISP and semiannual assessments; provides transportation; performs case management and maintains confidential files; utilizes agency computer programs for documentation; collaborates with families, guardians, and staff; attends meetings; adheres to regulatory and funding requirements. Performs duties with attention to safety in a residential environment and may assist with meal breaks and activities as assigned.
Required Qualifications
- BA/BS in Human Services /related field or associate degree plus 3 years related experience with persons with ID/DD or Human Services or HS Diploma/GED plus 5 years of related experience with persons with ID/DD or human services
- Valid driver's license from the state of residency
- Medication Administration
- First Aid
- CPR
