Residential Counselor (Teacher/Counselor) with Lifeworks for adults with ID/DD in a nonprofit residential program. Responsibilities include promoting health through routine and preventive care, direct care tasks (showering, toileting, hygiene, meal-time assistance), ensuring safe residential environments, providing education, training, counseling, and physical assistance to achieve ISP goals in areas such as functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication, community mobility, self-esteem, and money management. Develop, write, and present annual ISPs and semi-annual assessments in collaboration with staff and directors. Provide transportation as needed and communicate with family, guardians, and other stakeholders to ensure consistency. Perform case management duties including maintaining confidential files, documenting activities, and attending regular meetings. Employment is subject to required background checks and a valid driver’s license.
