Job Summary
Promote optimal health through routine, preventive, and emergency care while assisting with hands-on direct care tasks like showering, toileting, and meal-time support. Ensure residential environments remain safe and secure, then provide education, counseling, and physical assistance to achieve individualized service plan goals in academics, hygiene, and community mobility. Develop annual service plans and semi-annual assessments in consultation with staff, manage case files and documentation, and coordinate transportation as needed. Communicate regularly with family, guardians, and funding sources to maintain programming consistency. This role requires a Bachelor's degree or equivalent experience, a valid driver's license, and background checks, with a Thursday through Sunday shift schedule.
Required Qualifications
- Experience working with Individuals with ID/DD
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Valid Driver's License
- Excellent verbal and written communication skills
