Job Summary
Provide teaching, monitoring, and recovery-oriented supports including assertiveness training, medication management, and daily living skills. Ensure comprehensive person-centered planning for resident admission, ongoing services, and discharge. Observe, document, and report resident behaviors and progress while assisting with meal preparation, recreational activities, safe transportation, and household chores. Supervise resident medications and complete assigned duties to maintain cleanliness and safety standards. Full-time role in Rochester, NY, with a pay range of $22–$23.60 per hour based on education and experience.
Required Qualifications
- High School Diploma
- Clean driving record that meets Agency's Clean Driving Record Policy
- Must be at least Eighteen (18) years of age
- QMHS Standard
- Bachelor's degree in Human Services
- Associate's degree in Human Services
- 3-years FT related paid experience
- High school diploma and 5 years related paid experience
