Job Summary
Residential Coordinator assists in planning and implementing a rehabilitative, client-centered program and individually designed service plans for adults with mental illness and substance use challenges in a residential setting. Responsibilities include writing individual service plans and reviews, monthly progress notes, facilitating support team meetings, completing admission and discharge paperwork, daily progress notes, goal contacts, supervising medication administration, ensuring access to medical care, and linking consumers with vocational and educational opportunities within the community. Schedule is Monday–Friday with every other weekend, 7am–3pm, at the TTAP location in Kingston, NY. Requires a High School Diploma, strong writing, organization, interpersonal and computer skills, and a clean NYS driver’s license with 3 years driving experience.
