Job Summary
Initiate, support, and ensure quality of life through empowerment, safety awareness, and respect for individuals served by actively participating in Multidisciplinary Team meetings and accompanying clients to medical appointments. Oversee medication treatment and administration, including preparing Health Care Provider Orders, Medication Administration Records, and Weekly and Monthly MAP Checklists, while serving as a member of the on-call team for crisis intervention. Act as a primary implementer of strategies to meet Individual Service Plan (ISP) goals, taking necessary actions to protect individuals in the event of abuse or neglect complaints. Complete all required documentation within the electronic health record, including Health Encounter Forms and Contract Reports, and ensure proper review before submission. Maintain property safety by recognizing and reporting concerns, conducting required fire drills, and ensuring individuals receive benefits management for Social Security, health insurance, and housing subsidies. Provide direct and indirect services to promote independence while attending in-services, meetings, and trainings as assigned.
Required Qualifications
- High School Diploma or GED
- 3 years relevant experience
- Associate's Degree
- Bachelor's Degree
- Minimum of two years' experience in human services
- Valid driver's license
- Reliable car available for all working hours
- Good organizational skills
- Excellent interpersonal, verbal, and written communication skills
- Familiarity with internet, word processing, electronic file management, and email systems
- Regular and punctual attendance
- MAP certification

- Supervisory experience
