Job Summary
Arrange home visits, prepare transition plans, and coordinate independent living services while acting as the primary contact between schools and residential clients. Coach family visits, complete assessments including FASP and SPR, and maintain accurate case notes in the Connections database. Provide transportation for clients, maintain contact with DSS and families, and function as a contributing member of the interdisciplinary treatment team. Attend dorm meetings and treatment review meetings. Requires a high school diploma or GED with two years of social services experience. Onsite work with community-based travel.
Required Qualifications

- Bachelor's Degree
- two years of experience in social services or case management
- three years of social services or case management experience
- four years of experience in case management
- six to eight years of experience in case management
- Experience with individuals in care systems
- Must pass physical
