Residential Case Manager

Job Summary
Educate and refer patients to community agencies for food, shelter, and employment needs while conducting screenings, assessments, and client group facilitation. Manage Electronic Health Record systems for data entry and tracking, assist clients in establishing support systems, and act as advocates to improve service access. Provide input on treatment needs and disposition, participate in clinical staffing decisions, and complete documentation on provided forms timely. Collaborate with counselors and staff to promote cooperation and portray a professional image to the community. Requires a Bachelor's degree in human services, CAP preferred, and ability to work 40+ hours including before and after normal hours.
Required Qualifications
- Bachelor's degree in human services field
- Strong Microsoft Office Suite experience
- Ability to work collaboratively with others in a manner that is pleasant and professional
- Ability to work well in a team environment
- Ability to exercise good judgement and discretion
- Excellent verbal skills and written communication skills
- Ability to complete work tasks within scheduled work hours
- Willing to seek out new work projects and contribute ideas and efforts to the constant improvement of WestCare services
- A minimum of 40 hours per week
- Regular attendance
- Being able to work as many hours as is necessary to complete job tasks
- Successful completion of a pre-employment drug test
- Successful completion of a pre-employment physical
- Successful completion of a criminal background check
