Residential Case Manager

Provide supportive counseling and crisis intervention to residents while implementing individual service plans to achieve rehabilitation goals. Maintain accurate case records, progress notes, and service plan summaries, and order, supervise, and count medication in accordance with facility policies. Manage crises within the residence by utilizing de-escalation techniques, notifying supervisors, and documenting events on appropriate forms. Lead recreational and counseling groups, escort residents to community appointments, and conduct apartment inspections to support daily living skills. Monitor video surveillance, perform hourly building walk-throughs, ensure visitor identification, and conduct monthly fire drills. Perform light maintenance tasks including housekeeping and snow removal. Remain on site until relief staff arrive and participate in residential staff meetings.
