Job Summary

Develop and coordinate youth service plans, needs assessments, and discharge plans while maintaining Medicaid and licensing compliance. Build positive relationships with youth, families, and referring agencies through routine contact, on-site visits, and service plan meetings. Document care coordination, progress reports, and billing information to support treatment teams and ensure timely medical appointments. Provide administrative supervision to residential staff, assist in training new personnel, and lead discharge processes with legal guardians and placing agents. Arrange transportation for appointments and home visits in accordance with facility standards. Maintain up-to-date certifications in CPR, First Aid, and medication administration while driving as needed.

Required Qualifications

Master's degree in social work, psychology or counseling
Bachelor's degree in social work, psychology or counseling with ability to obtain QMHP
Bachelor's degree with ability to obtain QMHP
Valid driver's license and good standing with Elk Hill's auto-insurance policies
Up-to-date certification in CPR/First Aid
MANDT certification
DBHDS Trained Investigator Training
Up-to-date certification in the facility's Behavioral Management Program
Ability to apply Behavioral Management Program skills as needed and required in day-to-day interactions with youth
Valid certificate in an approved medication aide training class
Good physical health and remain current with annual TB screen and other certifications
Up-to-date certifications in approved courses for CPR/First Aid
Valid driver's license and good driving record
Must be trained in the REVIVE! Opioid Overdose Education and Naloxone Administration (or similar) training and be willing to administer naloxone in the event of a suspected overdose
Maintain DBHDS Trained Investigator training
Ability and willingness to physically perform all functions of the facility's Behavior Management Program
Ability to traverse inside and outside of the property, or other locations, including uneven terrain or stairs
Periodic transportation of a client as needed or required
Periodic transportation of other staff members as needed or required
Periodic travel to other locations as needed or required
Exceptional ability to develop and coordinate a youth's service plan and service planning needs
Strong awareness and knowledge of group dynamics and psychiatric diagnoses
Sound recordkeeping and case management abilities
Highly skilled at public relations
Essential are fluent oratory and professional writing skills
Knowledge and understanding of cultural diversity and the impact of various socioeconomic characteristics on our youth
The ability to support constructive family contact and involvement in community activities as appropriate
Ability to engage, support, and work effectively with others, including but not limited to the treatment team and other internal and external stakeholders
The temperament to work with, and care for, children and families with special needs
Sense of humor
