Job Summary
Conduct intakes, needs assessments, and create individualized service plans for program participants. Provide direct support, assist with legal matters, and ensure enrollment in insurance, food stamps, and other social service benefits. Connect clients with resources, transport them to community agencies, and prepare discharge proposals with follow-up. Develop community resource networks, lead educational groups, and document progress in electronic medical records. Prepare and distribute electronic reports on client data and service needs.
Required Qualifications
- Experience in case management
- Knowledge of wraparound services
- Possess strong interpersonal and communication skills
- Ability to present educational topics in group format
- Ability to develop community resource network
- Computer skills required including Microsoft excel, word, power point and online electronic medical record
- Must have valid driver's license and submit DMV record at time of hire
- Exhibit organizational and time management skills
