Job Summary

Coordinate participant transitions to independent living through counseling, encouragement, and progress documentation aligned with individualized rehabilitation plans. Conduct initial and annual assessments to develop person-centered recovery plans, while acting as a liaison with community agencies and families. Provide ongoing symptom assessments for mental illness to ensure appropriate interventions and assist in case conference presentations. Maintain secure case records for all assigned participants and participate in staff planning, training, and administrative meetings. Requires crisis intervention capabilities, direct care in a community-based setting, and weekend coverage on a rotating schedule.

Required Qualifications

  • Bachelor's degree in counseling, psychology, social work, nursing, vocational/psychiatric rehabilitation, education or other mental health field from an accredited college or university
  • Knowledge of mental health and substance abuse systems and supports
  • Ability to work independently
  • Good communication skills both oral and written
  • Possess a valid driver's license with an acceptable driving record
  • Direct experience in mental health services with emphasis on direct care in a community-based setting
  • Requires the ability to lift, carry, fold, bend, kneel, stack, stoop, which includes direct intervention with participants
  • Frequent talking and listening
  • Ability to climb and descend stairs
  • Possess manual dexterity and fine motor skills
  • Requires crisis intervention in emergency situations
  • May be required to report to work during emergencies including inclement weather
