Resident Services Coordinator

Conduct individual casework and case management for frail or at-risk residents, including assessments, developing individual case plans, applying interventions, coordinating services, and monitoring progress. Establish relationships with residents, families, and neighborhood groups through home visits, office visits, and ongoing contacts while identifying needs, assessing health status and risks, and serving as a liaison with outside social and health agencies. Provide information, referrals, crisis intervention, and counseling; maintain accurate records using charting software and the One System; and facilitate neighbor-to-neighbor, family, and volunteer care giving. Collaborate with the San Francisco Departments of Public Health and Human Services, housing managers, and community staff to address resident needs and implement programming.
