Coordinate and evaluate the Social Services program to meet residents’ psycho-social needs, including assessing resident needs based on admission history, health, psychological and social factors; promote a positive living experience for residents; maintain resident records; ensure residents are linked to appropriate supportive services; ensure confidentiality by adhering to HIPAA and privacy standards; collaborate with the community team to support independent living for residents.
Required Qualifications
- 1+ years of experience in case management
- 2+ years of social services experience
- A bachelor’s degree required in social work or related program such as gerontology, psychology, or recreation therapy
