Job Summary
Promote healthy lifestyles by managing resident care experiences through communication flow between the medical clinic, therapies, and interdisciplinary teams. Conduct initial and periodic reviews of residents to determine admittance feasibility and health needs, while monitoring service delivery for appropriateness and timeliness. Provide limited case management for residents requiring assistance beyond general community services and educate individuals and groups on available services, application procedures, and resident rights. Serve as a liaison between families, residents, and healthcare professionals, responding to emergency requests as part of the response team. Maintain individual resident files and coordinate assistance for those identified by management. Requires a Licensed Practical Nurse license with at least two years of senior adult experience, moderate physical activity, and ability to respond in crisis situations.
Required Qualifications
- Licensed Practical Nurse license, in good standing in state of residency
- Minimum 2 years working with senior adults, preferably in a Senior Care living community
- Able to effectively respond in crisis situations
- Must possess excellent customer service skills
- Able to read, write, and speak with English language
- Moderate physical activity required by handling objects up to 50 pounds occasionally and/or 20 pounds frequently
- Possible exposure to chemicals, blood borne pathogens and other infectious diseases
- Sits, stands, bends and moves frequently during working hours
- Is subject to frequent interruptions
- Interacts with residents, family members, visitors frequently
- Frequently interacts with relevant stakeholders including team members, developers, consultants, brokers
