Job Summary
Serves as primary counselor to a group of residents with psychiatric disabilities, developing, monitoring, and modifying individual service plans for goals like meal preparation, medication management, and community integration. Provides supervision of resident self-administration of medications, trains residents in money management and budgeting, and accompanies residents to appointments and outings as directed. Documents implementation of service plans, medical and behavioral conditions, and incidents using required forms, while maintaining confidentiality and alerting supervisory staff to safety issues or hazards. Uses preventive counseling skills to avert crises, practices safety awareness, and acts as an advocate for residents as assigned by case managers or supervisors.
Required Qualifications
- High school diploma, GED or ability to pass written TABE test
- CPR and first aid certification or the ability to complete certification at first opportunity presented by employer
- Valid and clean NYS driver's license
- Must participate in the NYS LENS program (License Event Notification) and remain compliant throughout employment
- Ability to lift up to 35 lbs
