Resident Case Management

Resident Case Management role responsible for evaluating residents, orienting residents and families, maintaining ties with family/community, and facilitating access to health, mental health, dental, and social services. Includes assisting with income entitlements, coordinating with primary medical care, arranging services and documentation, discharge planning, handling grievances, participating in quality assurance, policy development, staff education, and collaboration with hospitals or rehab centers for admissions. Also includes activities in admissions and marketing, developing referral relationships, and leadership of staff while maintaining regulatory compliance with NYS/DOH guidelines. Work location is one site in Cortlandt Manor, NY; equal opportunity employer.
