Resident Care Manager-RN

Resident Care Manager responsible for coordinating comprehensive resident care plans and communicating with residents and families about plans of care and treatment protocols. Manages daily workload to address changing resident needs, performs utilization review for appropriate and timely care, coordinates referrals and admissions/discharges, maintains accurate documentation, and promotes collaboration among residents, families, care teams, physicians, and payers. Requires ability to relate positively with residents, families, staff, and professionals; strong time management and organizational skills; intermediate computer skills; and compliance with licensing and continuing education requirements. RN/LPN/LVN credentials are required or preferred as noted; supervision responsibilities are not applicable to this position.
