Job Summary
Coordinate resident-directed programming and develop individualized service plans based on move-in assessments. Assess resident needs, participate in orientation, and provide leadership and education to care staff and families regarding best practices for dementia care. Mentor volunteers and conduct onboarding, while coordinating schedules and ensuring adequate coverage for care staff. Conduct interviews for new hires, deliver performance reviews, and complete nursing day-to-day duties on scheduled shifts. Promote physical and emotional well-being by encouraging resident activities and maintaining communication with families regarding care.
Required Qualifications
- Associate degree and 2+ years of experience preferred OR 4+ years of relevant experience
- CNA or CMT certification preferred
- CPR or BLS certification preferred
- Experience in a similar role, preferably within a senior living community or healthcare setting preferred
- Demonstrated proficiency in common digital tools and applications, including Microsoft Office programs (Outlook, Excel, Word), web-based platforms, electronic health record systems, and any HR-related systems as applicable
- This position may need to lift up to fifty (50) pounds independently, and up to 200 pounds with assistance
- Individuals in this position are required to follow all infection prevention and control protocols, including the use of personal protective equipment (PPE) when necessary, to ensure the health and safety of residents, staff, and visitors
- All offers of employment are contingent upon the successful completion of a background check and drug screening, participation in required health assessments (such as TB testing or physical evaluations), and the use of designated personal protective equipment (PPE) as required by company policy and applicable law
