Job Summary

Develop and facilitate resident-directed programming to meet assessed needs, interests, and capabilities. Assess residents upon move-in, prepare individualized service plans, and coordinate orientation while identifying potential changes or concerns. Provide leadership and education to care staff and families regarding individualized service plans and best practices for dementia care. Mentor staff and volunteers, conduct orientations, and coordinate schedules to ensure adequate coverage. Participate in nursing day-to-day duties on scheduled floor shifts and conduct or participate in care staff interviews and onboarding. Deliver performance reviews, implement new hire training, and actively participate in resident activities. Maintain excellent care standards across all levels, communicate effectively with families regarding physical, personal, and emotional wellbeing, and resolve conflicts as needed.

Required Qualifications

  • Associate degree and 2+ years of experience preferred OR 4+ years of relevant experience
  • CNA or CMT certification preferred
  • CPR or BLS certification preferred
  • Experience in a similar role, preferably within a senior living community or healthcare setting preferred
  • Demonstrated proficiency in common digital tools and applications, including Microsoft Office programs (Outlook, Excel, Word), web-based platforms, electronic health record systems, and any HR-related systems as applicable
  • This position may need to lift up to fifty (50) pounds independently, and up to 200 pounds with assistance
  • Individuals in this position are required to follow all infection prevention and control protocols, including the use of personal protective equipment (PPE) when necessary, to ensure the health and safety of residents, staff, and visitors
  • All offers of employment are contingent upon the successful completion of a background check and drug screening, participation in required health assessments (such as TB testing or physical evaluations), and the use of designated personal protective equipment (PPE) as required by company policy and applicable law
