Job Summary
Oversee day-to-day resident care operations and ensure continuity of care across shifts at Francis House, coordinating admissions, transitions, and care planning with hospice partners. Supervise caregivers, volunteers, and on-call administrators while preparing medications for unlicensed staff per NYS guidelines. Monitor resident needs, maintain accurate confidential documentation, and advocate for timely clinical communication. Provide supervision, guidance, and support to the team, fostering a collaborative environment aligned with the mission of helping persons with terminal illnesses die with dignity. Participate in quality improvement initiatives, incident reviews, and staff onboarding.
Required Qualifications
- Bachelor's degree in nursing
- RN licensure
- Minimum of 3 years of experience in hospice, palliative care, long-term care, or a related healthcare setting
- Prior supervisory or care coordination experience preferred
- Strong understanding of hospice and palliative care principles including patient care charts and histories
- Excellent communication, interpersonal, and conflict-resolution skills
- Ability to lead and support a multidisciplinary team in a compassionate residential environment
- Strong organizational skills and attention to detail
- Ability to maintain confidentiality and exercise sound judgment
- Basic computer skills including the ability to send/receive e-mail messages, perform basic word processing and/or data entry, and proficiency with electronic documentation
- Ability to respect the dignity of all who serve and are served
- Ability to represent Francis House effectively in a variety of settings and with diverse communities
- Ability to educate residents and/or families as to the nature of disease and to provide instruction on proper care and treatment
- Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
- Ability to observe, assess, and record symptoms, reactions, and progress
- Ability to develop, implement, and modify multidisciplinary healthcare plans
- Ability to make administrative/procedural decisions and judgments
- Knowledge of clinical and social services documentation procedures and standards
- Ability to sit, stand, walk, talk, hear, use hands to finger, grasp, handle or feel, and perform repetitive motions of the hands and/or wrists
- Push, pull, reach, stoop, bend, kneel and climb
- Exerts up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently or constantly to move objects
- Close mental and visual attention required to perform machine operations and set up, read, and document resident information
- Believes in and supports the Francis House mission, vision and values and supports its philosophy of care
- Sensitive to the needs of those in palliative care
- Maintains organizational and client confidentiality at all times
