Job Summary
Develop, update, and coordinate patient care plans to optimize outcomes and transitions across the care continuum. Provide skilled nursing services and administer medications in the home environment according to the facility's plan of care. Foster a learning environment for patients, families, nursing staff, students, and the community by facilitating formal and informal education. Align patient safety, self-safety, and environmental safety as equally important factors for best outcomes. Maintain proficiency in documenting and reporting on resident progress while adapting wellness programs to suit various physical abilities and cognitive levels.
Required Qualifications
- Previous experience in a similar role, preferably in a nursing home or long-term care setting
- Strong interpersonal skills and empathy when working with elderly residents
- Ability to create and adapt wellness programs to suit various physical abilities and cognitive levels
- Excellent organizational and communication skills
- Proficiency in documenting and reporting on resident progress
Desired Qualifications
- Relevant certifications in gerontology or wellness programming
