Job Summary

Assess residents' needs and develop individualized service plans in collaboration with clinical staff, residents, and families. Monitor and adjust care plans as needs change while ensuring compliance with regulatory guidelines. Provide leadership and training to caregiving staff to ensure adherence to protocols and support scheduling for quality delivery. Serve as the primary point of contact for residents and families regarding care concerns, facilitating meetings and communicating updates compassionately. Conduct regular audits and partner with the Executive Director and healthcare providers to maintain community compliance and coordinate care services. Requires Associate's or Bachelor's degree in Nursing or Healthcare Administration, active LPN or RN license, and minimum 2 years of senior living experience. Required Qualifications

  • Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting
  • Ability to manage multiple priorities in a fast-paced environment
  • Knowledge of state regulations and compliance standards for assisted living and memory care
  • Proficiency in electronic health records (EHR) systems and basic computer skills

Desired Qualifications

  • Associate's or Bachelor's degree in Nursing, Healthcare Administration, or related field preferred
  • Active LPN or RN license preferred (depending on state requirements)
  • Previous supervisory or leadership experience in a healthcare environment is highly desirable
  • Strong leadership, communication, and interpersonal skills
