Resident Care Coordinator (RPN) - Long Term Care

Job Summary
As a leader of our frontline care team, the Resident Care Coordinator plays a vital role in the Home. Your voice, ideas, and leadership help shape daily care practices, while your presence and energy contribute directly to creating meaningful moments and smiles for our Residents who live here. You will provide clinical leadership in your assigned resident home area as an RPN, lead and support the PSW team, ensure Resident-Directed Care principles, oversee clinical programs, participate in quality improvement activities, collaborate with leadership on educational needs, onboard and train new PSW staff, coordinate team meetings, manage performance matters, advocate for Resident Rights, maintain accurate documentation, participate in care conferences, stay current with best practices in gerontology, adjust hours as needed to support operations, and support the MRC and Nursing Department. If you have registration in good standing with the CNO, we will aim to respond within a week. Required Qualifications
- Registration with the College of Nurses of Ontario in good standing
- Vulnerable Sector Screening (VSS) check completed within the past 6 months
- TB 2 Step updated within the last year
- Experience in geriatric nursing with a genuine interest in all aspects of Long Term Care
- Knowledge of LTC legislation, MOH compliance and Health and Safety
- Excellent interpersonal, communication, organizational and team skills
- Demonstrated experience and passion for positive leadership
- Computer skills (knowledge of Point Click Care, Microsoft Office)
- Proficient in HQO principles and activities related to quality improvement activities including data collection, analysis, reporting, building QIPS.
