Job Summary
Coordinate resident care by completing MDS documentation, rounding with physicians, and overseeing care plan meetings. Serve as the wound care and Restorative Program coordinators while providing direct support to staff. Participate in physician rounds and assist direct care staff as needed. Gain accurate financial reimbursement through comprehensive care coordination. Requires two-year nursing degree, Wisconsin RN license, and wound care certification. Schedule includes 80 hours Monday through Friday from 8:00am to 4:30pm with no weekends. Required Qualifications
- Two-year degree in nursing
- Wisconsin Registered Nursing license
- Current CPR certification
- 6+ months of related work experience
- Intermediate proficiency with computers
- Exceptional clinical skills
- Strong oral and written communication skills
- Self-starter with excellent interpersonal communication and problem-solving skills
- No weekends
- Paid holidays
