Resident Care Coordinator (RCC)

Job Summary
Coordinate staff training, scheduling, and time off requests while managing payroll calculations and processing employee orientation. Audit staff performance, skills, and medication records, and oversee home health, hospice, and ancillary care providers to ensure proper documentation of visits. Act as a liaison between facility staff, outside agencies, and physician offices to manage resident needs and service plans. Maintain current talk lists, monitor medication rooms, and purchase supplies and durable medical equipment as directed. Participate in quarterly care conferences and QA activities to support administrators in maintaining fiscally sound operations and policy compliance.
Required Qualifications
- Two years prior experience in working as a supervisor, caregiver, or med aide
- Sufficient communication and language skills to enable them to perform their duties and interact effectively with residents and staff
- Education and or experience in management and/or supervision
- Strong communication skills both oral and written
- Knowledge of state RCF/Memory Care rules and regulations, and facility rules
- Have two years successful experience providing care to persons in a community based or long term care setting
- Minimum of two years education in a health related field
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from employees, groups of managers, clients, customers, and the general public
- Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
- CPR
- First Aid
- Food Handlers
- Current Driver's License
- Ability to occasionally lift and/or move up to 50 pounds
Desired Qualifications
- Two years prior experience in working as a supervisor, caregiver, or med aide is preferred
- Strong computer skills with a working knowledge of Word, and Excel are preferred
