Job Summary
Resident Care Coordinator serves as liaison between residents, families, and the care team to promote a safe living environment; supports Health and Wellness Director by assisting in scheduling, supervising staff, and ensuring systems are in place to provide quality assisted living services; coordinates care with team and external providers; assists with medication administration and direct resident care as needed; responsibilities include scheduling and assigning shifts, supervising care team, maintaining resident records, coordinating admissions and care planning with home health agencies, facilitating communication among residents, families, and healthcare providers, and addressing residents' needs and complaints; requires knowledge of care regulations and willingness to participate in weekend rotation.
Required Qualifications
- High School Diploma or equivalent
- Must meet the state requirements for medication administration in assisted living
- An LVN nursing license is a plus
- Current First Aid and CPR license or ability to obtain
- At least two (2) years of experience administering medications and providing care in a professional caregiving setting
- At least one (1) year of relevant supervisory experience, including staff scheduling
