Job Summary
Provide care and supports in an ADAPT residential facility, promoting independence and a homelike environment. Implement ADLs and activity goals, participate in interdisciplinary treatment plans, document progress and incidents, respond to crises, administer medications as AMAP under supervision, transport clients to activities, assist with meal planning and housekeeping, and uphold confidentiality. Foster dignity, respect, and person-centered approaches in all interactions while maintaining safety and reporting concerns per policy. Work as part of a team to support residents in achieving personal goals and high quality of life.
Required Qualifications
- High School Diploma or GED
- Ability to read, write, speak and understand English
- Valid New York State Driver’s License
- Ability to administer medications as AMAP under nursing supervision
- Strong interpersonal and communication skills
- Ability to respond to crisis situations and make decisions under pressure
