Job Summary
Renal Vascular Access Clinical Nurse Consultant (CNC) is a registered nurse with expert knowledge and clinical competencies in nephrology, collaborating with nephrologists, vascular surgeons, interventional radiologists, anaesthetists and the broader health team to assess and manage CKD patient care. Responsibilities include coordinating CKD program communications, monitoring CKD parameters to finalise treatment plans, providing education to patients and families on chronic kidney disease and renal replacement therapies, promoting ESRF treatment options including transplant and home dialysis as first-line, and facilitating referrals to dietitians, diabetes educators, social work, and local healthcare services. The role requires leadership and partnership with patients to support health promotion and continuity of care within SVHM's renal service, with a salary classification of ZJ4 (~$110,333.60 per annum pro rata) and CBD Melbourne location. This fixed-term, full-time position emphasizes career development, continuous improvement, and a strong focus on education, communication, and integration across nephrology care teams.
Required Qualifications
- Degree in general Nursing meeting the registration requirements of the Nursing and Midwifery Board of Australia
- Current Nursing Registration with AHPRA
- Post Grad Certificate in Renal Nursing
- Significant postgraduate nursing experience in Nephrology
- Comprehensive knowledge and skill in managing CKD pathways
- Current National criminal history check, or willing to obtain
