Job Summary
Counsel patients and families on Chronic Kidney Disease and End-Stage Renal Disease, adapting to lifestyle changes and renal replacement therapy. Develop therapeutic relationships while orienting patients to the treatment facility and care team. Provide psychosocial summaries to determine rehabilitation potential and explore community resources for referrals. Assist with transplant evaluations, home training work-ups, and follow-up for failed transplants. Conduct in-service training for staff and lead education programs for the community. Participate in patient care planning meetings, unit budget reviews, and Quality Assessment Performance Improvement processes. Supervise primary nurse staff and dietitians regarding psychosocial issues. Maintain records for social service statistics and prepare correspondence to agencies and medical providers.
Required Qualifications
- Master's degree in social work with a specialization in clinical practice from a school of social work accredited by the Council on Social Work Education
- At least 2 years as a social worker, 1 year of which was in a dialysis unit or transplantation program prior to September 1, 1976, and has established a consultative relationship with a social worker who qualifies under § 494.140 (d)(1)
- References indicating candidate's reliability, good work habits, emotional maturity, and ability to establish and maintain effective interpersonal relationships
- Knowledge of normal and abnormal human behavior and its relationship to illness and medical treatment
- Ability to relate to wide variety of socio-economic and cultural groups
- Ability to relate to, communicate with, and work with medical staff
- Ability to work in a patient care setting and interest in medical information in general, as worker will be closely involved with patient's treatment as well as care plan and must closely coordinate his/her work with these plans and the activities of the other team members
- Experience in working with terminal illness and the ability to feel comfortable in working with life and death situations
- Interest and ability in supervision, administration, research, community organization, and education of staff and others
- Ability to communicate clearly and concisely both orally and in writing
- Ability to convey directions in an easily understood manner
- Ability to express ideas effectively to supervisors, physicians, peers, and other clinic employees
- Ability to listen and respond in an understanding manner with staff members
- Ability to relate to people in a manner which instills confidence and establishes support
- Flexibility to adjust to changing conditions and demands of the job
- Ability to remain calm, interpret problems and conditions as presented, and to provide solutions under stress
- Moderately active work; walking and standing for considerable lengths of time
- Visual acuity to prepare and read written material accurately
- Audio acuity to listen attentively and accurately
- Computer skills with word, excel, and other appropriate software
