Job Summary
Remote Registered Nurse role providing primary geriatric care from a home office. You will collaborate with PCPs, Medical Directors, and Medical Assistants to deliver coordinated primary care to residents of senior living communities across California, Nevada, and Arizona. Key responsibilities include triaging inbound communications (calls, faxes, email, messages), supporting LVN/LPNs, performing chart preparation and patient education on care plan adjustments, managing post-appointment processes (diagnostic orders, referrals, medication orders), participating in daily huddles, conducting virtual patient assessments, and acting as a liaison among patients, PCPs, specialists, and other healthcare professionals. The role emphasizes crisis intervention, palliative care discussions, ongoing quality improvement, and research efforts to enhance geriatric care. Qualifications require an active California RN license, at least 2 years of clinical experience (geriatric experience preferred), and strong remote-work capabilities in a tech-enabled environment.
Required Qualifications
- Active, unrestricted California RN license
- At least 2 years of clinical experience
- Experience in geriatrics or related fields preferred
- Strong remote-work experience and tech proficiency
- Ability to collaborate with PCPs, MA teammates, and other healthcare professionals
- Experience with chronic care management and geriatric diseases/treatments
- Ability to provide patient education and care planning
- Comfort with crisis intervention and palliative care discussions
- Ability to participate in quality improvement initiatives
