Remote Physical Therapist - Contract

Job Summary
Conduct telehealth evaluations and develop individualized treatment plans for patients with chronic lung conditions, including COPD and ILD. Deliver 1-on-1 virtual treatment sessions focused on functional mobility, strength, endurance, and activity pacing. Write Plans of Care, communicate with referring providers, and monitor patient progress using the RTM platform to adjust programming based on data trends. Collaborate with respiratory therapists and nurse practitioners on complex cases while educating patients on breathing strategies and lifestyle factors. Document accurately using the EMR and Therapist Dashboard. Maintain minimum availability of 8 hours per week with consistent weekly scheduling.
Required Qualifications
- Active PT license in a PT Compact member state
- DPT from an accredited program
- Minimum 2 years of clinical experience
- Comfortable working independently with patients, including direct access cases without a physician referral
- Comfortable with or able to learn telehealth platforms, EMR systems, and remote monitoring tools
- Access to a computer with webcam and microphone, and reliable internet connection
- Minimum availability of 8 hours per week
Desired Qualifications
- Background in geriatrics, home health, cardiopulmonary, or outpatient
- Familiarity with outpatient PT billing
- Minimum availability of 16+ hours per week with consistent weekly availability
