Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, delivering sessions that support mobility, independence, and functional longevity. Document care effectively within FOX's EMR system and collaborate with interdisciplinary teams including OTs, SLPs, and Fitness Specialists. Incorporate telehealth and virtual care components when appropriate to extend access and flexibility. This role requires a valid Physical Therapy license, EMR proficiency, and the ability to perform patient transfers and drive to patient locations. Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
Desired Qualifications
- Experience with telehealth and other forms of virtual care
