Job Summary

Conduct high-volume outbound telephone outreach to patients to introduce Evergreen's pharmacy service, establish rapport, and guide them through enrollment. Deliver standardized messaging using approved scripts to manage patient questions and objections, then obtain and document consent by phone. Update the electronic health record by changing preferred pharmacy settings, triggering electronic prescriptions, and accurately recording consent or decline decisions. Manage outreach queues and follow-up cadences, completing multiple contact attempts per patient based on urgency and targets. Accurately document all call activity, outreach attempts, and coordination efforts in Evergreen's clinical and operational systems while adhering to HIPAA privacy standards. This 6-month contract with potential conversion to full-time employment supports a value-based kidney care model that connects patients with specialized mail-order pharmacy services and pharmacist-led clinical support.
